There are two medical/insurance forms listed below that need to be completed and returned by July 20, 2011. All forms must be satisfactorily completed (as deemed by the athletic training staff) in their entirety, with appropriate signatures and dates, before you will be allowed to participate in any team practices this school year. The two forms that are required of all returning student-athletes is the Emergency Information Form (EIF) and the Insurance Coverage Verification Form (ICVF). These two forms are linked below along with a letter that helps explain and answer any questions that you may have.
