Welcome V-Hawk athletes to start of the 2010-2011 school year. In order to participate in collegiate athletics at Viterbo University, all student-athletes must have the appropriate medical forms on file with the V-Hawk athletic training staff. All forms must be satisfactorily completed (as deemed by the athletic training staff) in their entirety, with appropriate signatures and dates, before you will be allowed to participate in any team-related activities this school year. These medical/insurance forms should be returned to the Viterbo athletic training office by July 21, 2010. Returning Student-Athlete (any student who has participated on a V-Hawk athletic team in years prior)

New Student-Athlete (incoming freshman, all transfers, and any one who may have been a Viterbo student but have not participated in collegiate athletics)
